Forms

To help expedite your check in time, we have various forms you can print and fill out prior to your appointment.  For new clients, please fill out our New Client Check In Form.  If your pet will be boarding with us, please fill out our Boarding Check In Form.  If your pet will be having dental work performed, please fill out our Dental Consent Form.  If you have any questions while filling out these forms, please call our hospital or ask upon your arrival.  We look forward to your visit!

Patients are seen by appointment or can be dropped off if you cannot stay for an appointment. Boarding pets can be dropped off and picked up whenever the hospital is open. Emergency cases shall always receive top priority.

We require full payment at the time that services are rendered. Good-faith estimates for hospitalization will be provided for pets requiring hospitalization or surgery and will be revised as needed. A 50% deposit on hospitalized estimates is requested. For your convenience, we accept Visa, MasterCard, American Express, Discover, Care Credit, cash and personal checks.

For your protection, and that of others, all dogs must be on a leash and properly controlled while in the waiting area or exam rooms. All cats must be presented in an appropriate cat carrier or on a leash.
